- When you purchase an individual or family policy from one of the above companies you are taken directly that companies secure web site and are choosing a plan directly from that company. In the case of AmeriHealth the policy is done manually and you mail in your application where a licensed agent will look over the application and submit the application if all appropriate information is included.
- The rates and quotes in the plan you choose are for the effective date you have chosen and your primary address only, and the rates or quotes may not be guaranteed and may change.
- Some insurance companies require dependent children to apply separately for their health insurance once they reach a certain age. These rules depend on the state you live and/or the policy of the insurance company you choose.
- Any copays, deductibles, or coinsurance are what your costs will be for those benefits.
- Hospitals, Doctors, and other health professionals are known as providers and the insurance company you select can not guarantee that any provider will stay in their network. Please check with the insurance carrier to see if your provider is in their network.
